Coronavirus Vaccine Passports 

From 17th May you will be able to show evidence of Coronavirus vaccination status for foreign travel to certain green list countries.  Please DO NOT contact the surgery for proof of vaccination status. Please download the NHS application (https://www.nhs.uk/using-the-nhs/nhs-services/the-nhs-app/) onto your phone where you will be able to see your vaccination record.  If you are not able to download an application you can request a paper copy of this information by calling 119 five days after your 2nd vaccination.

The vaccination role out is continuing and we now in a position to invite those aged 32yrs upwards without other health conditions.  Please get in in touch with the surgery if you are 32yrs or older and you want to have the vaccine - please call the surgery after 11am to arrange this or  0161 947 0770 or 0800 092 4020

It is important we have up to date contact information (mobile number if possible) so we can text you when vaccination appointments become available. You can update your mobile number here 

If you receive a text to book an appointment at Wythenshawe Lifestyle Centre, M22 1QW then this is from the surgery and not a hoax, please go ahead and book an appointment.  Please do not turn up without an appointment - you will not be vaccinated.

Online services

GP online services allow you to access a range of services via your computer, mobile or tablet.  Use the forms above to directly access advice and deal with queries without calling up.  Sign up to the NHS app or patient access for repeat prescriptions. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results
  • Patient access will allow you to book video consultations in advance

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

Alternatively you can get an access code for patient access from the reception team. 

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up
